Almonte General Hospital and Carleton Place & District
Memorial Hospital created the Mississippi River Health Alliance (MRHA) in 2016
to formalize their commitment to work together to improve each patient and
resident’s overall health care experience through a strong, coordinated system
of care. An integrated Senior Team leads the two Hospitals, as well as Fairview
Manor and Lanark County Paramedic Service which are operating divisions of AGH.
Over 600 caring staff, physicians and midwives provide excellent primary, acute
and long-term care to citizens of Lanark County, West Ottawa and Renfrew
County.
Job Description
The successful incumbent will work in consultation with
management and interdisciplinary team members, developing processes that ensure
data integrity, efficient data collection process, assessment completion and
data utilization. The RAI Coordinator
will correctly code assessments and provide ongoing support for validation of
coding while enforcing provincial and regional regulations, standards of
practice and interRAI requirements. Generates
RAI MDS 2.0 reports and facilitates their use in clinical, financial and operational
decision-making. Provides ongoing
leadership, mentoring, education on data accuracy, data submissions and
computer applications of the RAI process and coordinates a data quality program
to ensure data accuracy and promotes capacity building and staff competency.
Qualifications
· A minimum of five (5) years clinical experience
in a Long Term Care or Geriatric setting.
· Must have a minimum of 3 years’ experience with
RAI MDS.
· Registered Practical Nurse (RPN).
· Possesses a certificate of competence in good
standing from the College of Nurses of Ontario.
· Proficient documentation in care plans.
· Strong problem solving skills with an aptitude
for detail oriented work.
· Experience with MDS-RAI reporting software and
MOH data reporting.
· Excellent time management skills with ability to
meet deadlines.
· Proven leadership skills with strong problem
solving capabilities.
· Knowledge and experience with adult education
strategies/facilitation.
· Effective communication skills and ability to
work independently and collaboratively are essential.
· Proficient computer and presentation skills.
