Reporting to the Clinical Manager, the Care Facilitator provides leadership and organizational support to ensure effective and efficient day to day operations and optimize patient care demands on the all clinical units. The incumbent shall support the nursing staff to work collaboratively with the interprofessional team to enhance patient care. Additionally, he/she shall support the nurses to foster a culture of continuous performance improvement.

Qualifications & Experience:

Mandatory

·         Registered with the College of Nurses of Ontario (CNO)

·         Bachelor of Nursing

·         Minimum of 3 – 5 years recent relevant clinical experience

·         Excellent communication skills (oral and written)

·         Excellent organizational, interpersonal, problem solving and decision making skills

·         Demonstrates knowledge and ability in conflict prevention and resolution

·         Demonstrated leadership and organizational abilities and is able to apply in a clinical setting

·         Demonstrated computer competency

Preferred

·         Registered Nurses Association of Ontario (RNAO) Membership

·         Experience with clinical documentation in an Electronic Health Record

·         Canadian Nurses Association (CNA) and/or other related certifications

·     Membership in Specialty related Professional Association
